PRIDE Health is seeking a travel nurse RN Home Health for a travel nursing job in Silverdale, Washington.

Pride Health is hiring  Home Health RNs  to support our client in  Silverdale, WA 98383.  This is a 3+Months assignment (Possibility of extension or conversion). opportunity and a great way to start working with a top-tier healthcare organization!

 

Position Summary:

Primary functions of the Registered Nurse are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency and assume the responsibility for coordination of care.

 

Essential Duties and Responsibilities:

• Assess and monitor home health or hospice patients, developing and updating individualized Plans of Care based on physical, psychosocial, and environmental needs.
• Maintain accurate and timely documentation of patient care, ensuring quality, continuity, and compliance with regulatory and third-party payer requirements.
• Educate patients and families on medications, care techniques, safety, and self-care based on age, condition, and readiness to learn.
• Supervise and guide support staff (HHA/STNA/LVN) and coordinate referrals to other healthcare agencies as needed.
• Perform on-call duties, participate in interdisciplinary team meetings, and communicate effectively with all parties involved in patient care.

 

Required Knowledge, Skills & Abilities:

• Effectively assesses and responds to patient and family needs in diverse care settings.
• Demonstrates strong communication, organizational, and critical nursing skills, including wound care, phlebotomy, and IV therapy.
• Competent in using and maintaining medical equipment, catheters, and administering respiratory therapy.
• Maintains current licensure, confidentiality, and meets continuing education requirements.
• Works independently and collaboratively, adapting to flexible schedules and local travel.

 

Required Qualifications:

License/Certification:

• Current state license as a Registered Nurse - Required
• CHPN certification - Preferred
• Current BLS certification - Required
• Current driver's license - Required
• Must have reliable transportation and insurance - Required

 

Experience:

• Homecare Homebase Software Experience - Required
• OASIS Experience - Required
• 1-2 years' experience as a Registered Nurse in a clinical care setting - Required
• 2+ years' Home health experience - Required.

 

Education:

• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required
• Bachelors of Science in Nursing - Preferred

 

 

Location: Silverdale, WA 98383\nTravel Locations: North and South coverage needed- north is Bainbridge isle, Kingston, Hansville Poulsbo areas and south is Bremerton, Port Orchard and Olalla areas\nJob Type: 3 months  Contract\nSchedule: 8 hrs/Day – Day shift\nExpected hours: 40 hours in a week\n\nLocal Pay range: $45-$51/hour\n*Offered pay rate will be based on education, experience, and healthcare  credentials.\n\nTravel Gross Pay: $2400 - $2520/Week\n*Offered pay rate will be based on education, experience, and healthcare credentials.
