Job Requirements

• ASCP certification required
• Primary Source Verification of professional license and/or certification within 30 days of start date and updated upon expiration
• MA Conditional Licenses not accepted
• MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break
• MGB Massachusetts CORI background check within 30 days prior to start date and updated if >6 month break
• MGB Resume including past 7 years of work experience with city & state
• 2 recent references, minimum 1 Supervisor or Above, in Skillset
• MGB Skills Checklist required for submission
• MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions
• MGB Education Verification of highest level education achieved
• MGB SSN Trace listing addresses/alias for past 7 years
• MGB National Sex Offender Check within 30 days of start and updated annually
• MGB Exclusion Checks within 30 days of start and annually

Schedule Information

• Contract length is 13 weeks

Additional Details

• Documentation of two measles vaccines, two mumps vaccine, and one rubella (German measles) vaccines or two MMR vaccines or proof of immunity via blood test
• Hepatitis B vaccination record or proof of immunity or declination (strongly preferred)
• Influenza vaccine required during flu season Sept 15th - April 30th with no exemptions or waivers
• TB skin test screening within 3 months of start date and updated if >6 month break or negative QuantiFERON TB Test or T-spot within 3 months of start date and updated if >6 month break
• Physician Health Statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break
• Tdap vaccine within 10 years and updated upon expiration (preferred)
• Respirator Medical Clearance Attestation required if fit test is required for skill
• Positive PPD requires negative chest x-ray documentation
• MGB NWH Self-Study Orientation & Attestation available in Document Library
