We are currently seeking a Full Time RN Case Manager for the Bridgeport through East Haven Areas

Requirements:

• Minimum 1 year Home Care Experience
• Comprehensive knowledge and skill in current nursing practice.
• Knowledge of the concept of team and primary care nursing and the implementation of both models of care delivery.

Knowledge of the OASIS process.

• Knowledge of Community Resources.
• Ability to make meaningful observations, evaluations and reports.
• Ability to recognize the needs and concerns of patients, caregivers, and the multidisciplinary team to maintain therapeutic relationships.
• Ability to provide skilled nursing care based on scientific principles, the nursing process, basic behavioral concepts, and intelligent professional judgment.
• Ability to read, write and speak the English language in an understandable manner.
• Ability to bend, push, pull, move and/or lift
• Must possess a Nursing degree from an accredited College or University and be a Graduate of a CT approved RN program.
• Valid driver's license/Car insurance
• Active RN License in the State of CT
• Proof of Covid 19 Vaccination
