Southview Acres Health Care Center is recruiting for an MDS Coordinator to become a part of our team. As an MDS Coordinator, you will be responsible for overseeing and assuring the completion of the RAI/MDS process. This position is responsible for Medicare and MDS process.

Additional responsibilities of the MDS Coordinator include:

• Conducting Patient Assessments
• Developing Patient Care Plans
• Making Patient and Facility Recommendations
• Overseeing Medicare and Medicaid Programs
• Providing Facility Support

MDS nurses are vital to the operations and patient care practices of long-term care facilities, particularly those that receive federal funding or reimbursement.

Qualifications and skills include:

• Associate's degree (Bachelor's preferred)
• Licensed as a Registered Nurse through the State of MInnesota

• MDS certification
• Medical assessment
• Patient care
• Attention to detail and critical thinking
• Organization skills
• Communication skills
